After clearing the game once on any difficulty, the Secrets of Defense will be put in the Item Box on subsequents playthroughs. Like the Essence of Defense, it reduces the damage received when blocking attacks and needs to be in Ethan's inventory for it to function. It can be paired with the Essence of Defense to maximize its effect.
