At the beginning of the Siege of Anvil Gate, Pereira was wounded with a major abdominal wound when his artillery emplacement was hit by an RPG. Dr. Salka and a medic attempted to stop the bleeding and patch him up, but they were unable to stop the hemorrhage and Pereira died.
