I'm not sure about the bandage on Ichigo's cheek, but the scene where Rukia is wearing a blue dress while knocking Ichigo's "soul" form out of his body is seen in the Bleach OVA [Original Video Animation] "Memories in the Rain". It is separate addition to the original series.
