In the Old Man Logan storyline, The Hulk had inbred grandchildren who enjoyed killing people, so it would be safe to assume he was dead there, but they never said how. In one of the realities visited by the Exiles, the Hulk was killed by a setting off a concentrated gamma bomb a few feet away from him.
