On the Sheridan's maiden flight she experienced a critical overload in her warp drive system and all back-up systems had failed to correct the problem. It was decided to eject the ship's engine pods because an uncontrolled matter-antimatter mix was underway and couldn't be stopped, but unfortunately, the ejection systems also failed to activate and the Sheridan was destroyed in the subsequent explosion.
