This card will sometime proc (between 4% to 8%, usually) with the use of standard melee attacks against an enemy. It does not proc with the use of special attacks or additional free strikes. In addition, unlike most similar trinkets and enchants, the proc chance seems to be variable depending on the weapon speed, adjusting itself to give a standard rate of healing per minute, and not a standard proc chance per hit. The proc chance also seem to take into effect the modified speed and not the weapon default, so speed-increasing skill, such as Slice and Dice, seem to actually lower the proc chance per hit (while still keeping with the same ratio of healing per second).
