Jeremy Bentham's body release form sat clipped to a clipboard on top of his coffin in the mortuary of Hoffs/Drawlar Funeral Parlor. It was left unfilled, except for his name, as there was no one to claim Bentham's body. Jack Shephard removed the release form from the coffin before opening it to look at the body, and tossed it onto a stainless steel preparation table.
