- A powerful floating demon, similar to the Cacodemon. Both share the same basic round form, a singular eye and a large mouth cavity. Unlike the Cacodemon however, the Pain Elemental attacks by spitting an endless barrage of Lost Souls from its mouth. The mechanisms of this peculiar attack are entirely unknown and resist rational explanation. Upon expiry the demon is known to explode, releasing more Lost Souls. In addition to the Cacodemon, other similar (but weaker) demons include the Beholder and the Rahovart. An alternate type of Pain Elemental with two mouths has also been observed; this form has a small tail and long black hair on the back, but no arms. Due to the dual mouths, these Pain Elementals have an improved "rate of fire" making them the most dangerous incarnation of this demon
