Karai (下雷 Descending like Lightning) is a title of sorts, comparable to the title Kenpachi (剣八, Sword-Eight). To those who bear the title however, they lose their individuality of sorts, as the original bearer of the title was a Shinigami who is well known throughout history, but originally presumed dead. Only one person every century appears bearing the title, claiming to be the original Karai, with no proof to the contrary.
