An Exquisite Surmia Carving seems to be a carving of a Titan, and is obtained from the Murakai's Chest at the end of the Cathedral of Flames Dungeon after defeating Murakai, Lady of the Night. It is taken by Gron Fierceclaw, when you accept the reward for completing the Temple of the Damned quest.
