We continue to struggle with regulation, however we manage to stay under under 250 most of the time, and sometimes get down around 100. It took 9 units bid to get him into this range (he weighs 23 pounds), we are now giving 7.5 to 8 units bid. 3.) Are you feeding your cat a low-carbohydrate diet? 4.) Has your cat gone off of insulin? No. No way.

Yes, Purina DM -- and I think this is a key element in success with Lantus. Wet food vs. dry seems to make a big difference. When he is allowed dry DM, his BG runs higher. 4.) Has your cat gone off of insulin? No. 5.) Are you considering changing to or from Lantus? No way.
